As parent or guardian for the above-referenced child, I give my permission for my child to participate in Cheshire Baseball, also known as Cheshire Aviators, and acknowledge the risks involved with participation, including but not limited to injury. I, on behalf of my child, waive all claims relating to my child’s participation against the organization, and assume all risks associated with participation, including transportation to and from activities.
As parent or guardian for the above-referenced child, I acknowledge that I am responsible for setting a positive example for my child and I will conduct myself in an appropriate manner at all games and practices. The purpose of this league is to develop players of all skill levels in a supportive manner. I will encourage positive feedback, discourage bullying, and conduct myself as a positive role model. Failure to do so may result in removal from the program.
As parent or guardian for the above-referenced child, I acknowledge that I will not register my child for other baseball leagues during the season of January – June of the year your player is enrolled. Cheshire Baseball, also known as Cheshire Aviators, has a specific plan as it relates to protecting its players and does not allow players to play on multiple teams at the same time.
Cheshire Baseball, also known as Cheshire Aviators, does provide excess insurance coverage for medical expenses required due to injury to your child. In order to obtain coverage under this policy, you must first exhaust all other benefits provided by any applicable insurance policies that provide coverage for your child prior to submitting a claim on behalf of your child.
